Now here's what you do. First, photoshop a Doctor's name and facility within your area. For the phone number, get a Google Voice Account and create a phony automated messaging service that states the Doctor's name and that no one can come to the phone at the moment. IMPORTANT: make sure at the end of the message it says, "If this is an emergency, please hang up and dial 911." This should probably be enough to convince your professor if he calls. If not, then get a friend to call the professor back pretending to be a receptionist. Never impersonate a doctor. Also, have a friend write the note for different hand-writing or type it. Your call.

In Australia general practices are in majority all accredited by AGPAL, Australian General Practice Accreditation Limited, which uses this logo.

I would assume there is similar in the US, I have just done one quick google search and found something called The American Board of General Practice and their logo. You probably have a larger variety of entities that give such accreditation but its all really rather simple.
If you are paranoid about giving something completely bogus, go have a look at your local DR's front window or perhaps inside reception they will have their accreditation certificate displayed. Note the name of that company and its logo, use the name of a real DR at that practice on the letter and if you are convinced that for some reason anyone would actually bother checking... make an appointment at that DR on the date you put on the letter so they have some reference of you for that day. I can't imagine anyone needing to go to these lengths unless whomever you need to give this to is already looking at you with suspicion.
