They generally go for WAAYY higher than they do on the streets. Like, even bulk adderall goes for $18 a pill, and oxy 30s go for $40 or so. Like straight up rip offs. However, you can get illicit substances for reasonable or even cheap prices.

Well, wouldn't say they are rip offs as they sell fast as hell. Its expensive to source that shit. Generally, you have two types of vendors - one is your small time, he has access to one or two people's scripts directly who get it scripted from a doc, get insurance to cover it. These vendors can generally undercut the competition but they run out fast. But they have fat margins.
The other type is the one who bulk sources the scripts. There are a few ways to do this, but the most common is the aggregation of scripts and it usually goes through a middleman or two before it hits the DNM vendor. There is a person, the direct sourcer, he takes elderly people to a crooked doctor, who scripts for cash. The sourcer will pay these pay say $50 for their time, the doctor often charges $150-200 per patient per month for say the standard 120 10mg Norcos. B/c the doc is crooked, a legit pharmacy won't fill it. So they go to a pharm associated with the pain clinic. This script might run close to $200-300 to fill at the pharmacy for the 120 pills. It might even be more now for Norco now that it's schedule II. Its been a while since I've priced pain clinics. But on the upper end, we are already at $550 for the 120 norcos. Almost $5/pill. And it hasn't hit the DNM yet. This sourcer probably sells to your DNM vendor for $7-9/pill. Then the vendor sells to you for $10-15/pill for Norco. Same story with any other opioid.
More sophisticated vendors source at the supply chain level or might even have an affiliation with a crooked pharmacy. In either case, there are these little pharmaceutical distributors who sell to pharms. Often they do have requirements, if they want to pretend to be legit, that the pharmacy have a 50% makeup of opioid orders and 50% non-pain related orders. This is one reason why crooked pharmacies are expensive to fill at, often their suppliers pinch them like that. Or if they have an in with one of hte distributors, that'd be a healthy supply right there. The whole thing is quite intriguing.
I've seen couriers deliver these pills to pharmacies. I laugh thinking that this guy is carrying something more valuable than gold itself, on a mg per mg basis. yet, he is just walking in, incognito, like he's delivering water to an office.
