This is something I've wondered about as well. A couple months ago I posted that my initial Alphabay account got jacked - not because of phishing, either - I promise.
Well, as an idiot new user guess what I had done when setting up all these DN accounts?
My Abraxas, Nucleus, and Alphabay account names/passwords were all exactly the same.
I have a theory that potentially usernames/passwords are getting compromised somewhere due to a security flaw.

I like this theory. I remember when EVO went down, there was a guy on here claiming he had the database and was posting usernames and unencrypted passwords. Could be someone like that going through a database and trying those username/pass combos on other markets. But I honestly just believe its AB themselves doing the scamming. They endorse carding and fraud, and have Trappy as a main mod. Those two facts right there are enough for me not to touch that market with a ten foot poll.

I had 2FA enabled. Was not Phished. My vendor account was banned due to some very fishy claims from a person that I disputed.
Just like that, it's all gone!

And/or probably didn't use 2FA.

Every password I use is always unique and a burner password.
Pretty reliable theory. Almost every post has ended with them mentioning they never enabled 2FA. I think if your theory isn't correct then perhaps there is a bug in their code somewhere that allows dumping of usernames and password hashes, and maybe their passwords aren't salted properly or at all, and just maybe this exploit is very low key and only a few people know it to ensure they don't fix it.
